ASSUMPTION OF RISK AND RELEASE OF LIABILITY

By signing this document, I acknowledge and agree to the following:

  1. Voluntary Participation:
    I understand that participation in martial arts, including but not limited to striking, grappling, physical conditioning, and sparring, involves physical contact and inherent risk of injury, illness, or even death. I voluntarily participate at my own risk.

  2. Protective Equipment Acknowledgment:
    I understand that proper protective gear (including but not limited to mouthguards, gloves, shin guards, headgear, and groin protection) is required during sparring and contact drills. I agree that it is my sole responsibility to wear all required protective equipment and to ensure it is in proper working condition and worn correctly. Failure to do so may increase the risk of injury, for which I accept full responsibility.

  3. Medical Clearance:
    I affirm that I am in good physical condition and am not aware of any medical conditions that would prevent me from safely participating in martial arts training. I acknowledge that it is my responsibility to seek medical clearance before participation if I have any health concerns.

  4. Liability Waiver:
    I, on behalf of myself, my heirs, and legal representatives, hereby release, waive, discharge, and hold harmless Strike Martial Arts LLC, its owners, instructors, staff, agents, and affiliated parties from any and all liability, claims, demands, or causes of action for personal injury, illness, property damage, or death arising out of or related to my participation in activities at the facility or any Strike Martial Arts-related event.

  5. Conduct and Responsibility:
    I agree to follow all safety rules, guidelines, and instructions provided by Strike Martial Arts staff. I understand that failure to do so may result in injury or removal from participation.

MEDIA RELEASE

I grant full permission to Strike Martial Arts LLC to use, reproduce, and publish photographs, video footage, and audio recordings of myself/my child participating in martial arts classes, demonstrations, competitions, or related events for promotional and commercial purposes, including but not limited to social media, website content, and marketing materials.
I waive any right to inspect or approve the finished product or any compensation for use.

PARENT/GUARDIAN CONSENT (IF UNDER 18)

I, the undersigned, am the parent/legal guardian of the participant named above. I have read and understand this waiver and agree to its terms on behalf of the minor participant
